At Advanced Imaging San Marcos (AISM), we will make every effort to ensure that you have a positive and efficient experience while visiting our clinic. For your convenience, and to shorten your wait time upon arrival, the questionnaire forms required for your procedure are available to download on our website.
Upon arrival at our clinic we will require orders from your referring physician’s office, your drivers’ license or personal identification card and your insurance information.
Some referring doctors request that their patients leave our clinic with films or a CD of their procedure results. Please check with your referring physician prior to your appointment with us on his or her policy.
AISM works with your referring physician’s office to obtain the necessary insurance authorization prior to your scheduled appointment. A payment may be due at time of service.
You will receive a courtesy reminder phone call one day prior to your appointment date. We will inform you of any preparatory instructions at this time.
